Physical Therapy

Our Physical Therapy Services offer assistance with mobility, movement control, equipment assessment, home exercise instruction, evaluation and treatment of acute soft-tissue injury, evaluation and treatment of sports medicine injury, balance and ambulation training, flexibility and strengthening, and manual therapy.

Occupational Therapy

Our Occupational Therapy Services offer assistance to patients to gain and perform daily activities across their lifespan. Services may involve evaluations to determine the patient’s goals, assistance to improve basic motor functions and reasoning abilities, and education and training to assist with compensation for permanent loss of function. Our goal is to help patients have independent, productive, and satisfying lives.

Speech-Language Therapy

Our Speech Therapy Services offer assistance with the actual production of sounds, whereas our Language Therapy Services offer assistance with difficulty in understanding or putting words together to communicate ideas. Speech services may involve help with producing sounds in syllables or saying words, assistance with stuttering, assistance with the pitch, volume, or quality of the voice, and assistance with difficulties with drooling, eating, and swallowing. Language services may involve assistance with difficulties understanding or processing language and assistance with difficulty putting words together, limited vocabulary, or inability to use language in a socially appropriate way.

Medical Social Services

Our Medical Social Services offer help to individuals, couples and families coping with the social, psychological, cultural and medical issues resulting from an illness. Services may involve providing health-care resources, assisting with post-hospital needs, explaining to patients the epidemiology of their disease or illness and possible complications, assisting with advance directives and help with other long-term care issues. Our goal is to ensure that our patients’ medically related emotional and social needs are met and maintained throughout their medical treatment.

Dietitian Services

Our Dietitian Services focus on proper food and nutrition in order to promote good health. Services may include supervising the preparation and service of food, developing modified diets, and educating the patient on good nutritional habits. Our goal is to provide medical nutritional intervention, and to obtain, safely prepare, and advise on flavorsome, attractive, and nutritious food for our patients.

TeleHealth Homecare Visits

Medi Connect is our TeleHealth Homecare Division offering TeleHealth Homecare Visits. Our TeleHealth homecare solutions are designed to strengthen the connection between us, the healthcare provider, to our patients. With this tool, we are able to help patients who want more independence and control over their medical challenges. Our TeleHealth visits enable automatic and personal feedback, established by our clinicians and based on both biometric measurements and patient assessments in the home. During each session, our patients are instructed to measure their vital signs, respond to health assessment questions, receive timely, specific, and educational information via video, see motivational messages and complete surveys.

TeleHealth Monitoring and Scheduling Services

Medi Connect is our TeleHealth Homecare Division offering Remote Triage Monitoring and Tele-Scheduling services to medical providers. With our care coordination team, we Connect the patient to the provider by scheduling tele-video appointments. We carefully sync the TeleHealth equipment, Connect the right staff to fulfill the needs of the patient, set up the details associated with the tele-appointment, and ensure the delivery of quality care to the patient. Our services are ideal for patients with high hospital re-admissions, living in remote locations, or who have limited access to travel to a health facility for follow-up care. For providers, our services are ideal to assist them with maximizing their medical resources and promoting quality care through routine monitoring and coaching.
